Qualified Social Worker - Community Team

You will be working within a well respected Local Authority based in the Worcestershire area. The manager is looking for an experienced social worker to undertake assessments and carry a caseload. You will be arranging Package of care, Direct Payments, Extra Care Housing, Supported Living and Residential/Nursing homes.
