Tempting Recruitment are working alongside our client, a Local Authority, who are looking to recruit an experienced Care Home Trusted Assessor to join their team based in South London JOB DETAILSPAY RATE: £53 per hour WORK TYPE: Temporary WORK PATTERN: Full TimeLOCATION: South London JOB SUMMARYAn exciting opportunity has arisen for an experienced health or social care professional to join the Hospital Integrated Discharge Team as a Care Home Trusted Assessor.This pivotal role is central to reducing delayed transfers of care from acute hospital settings by ensuring timely, safe, and effective discharges to care homes. Working collaboratively with hospital multidisciplinary teams, care home providers, and system partners, you will undertake trusted assessments on behalf of care homes, support discharge planning, and remove barriers that delay patient flow.As a specialist in hospital discharge and care home assessment, you will play a key role in improving patient experience, clinical safety, and system-wide efficiency while representing care home providers within the discharge pathway.JOB DESCRIPTIONThis is an exciting opportunity to influence best practice in integrated care, improve processes, and deliver measurable financial and qualitative benefits across the service
  • Undertake trusted assessments and re-assessments of patients deemed medically fit for discharge into care home settings
  • Act on behalf of care home providers, supporting and streamlining their assessment processes
  • Liaise closely with hospital discharge teams, MDTs, care homes, district nurses and specialist services
  • Ensure discharge documentation, medication, and care plans are complete and accurate, supporting initiatives such as the Red Bag Scheme
  • Identify and resolve barriers to discharge, including equipment, communication or care planning issues
  • Represent the views and needs of older people, ensuring mental capacity and best interest principles are upheld
  • Monitor patient progress following admission from care homes and maintain effective communication with all stakeholders
  • Provide high-quality data, analysis and reporting on assessments, discharges, bed days saved and outcomes
  • Produce monthly monitoring reports and contribute to quarterly evaluations and case studies
  • Support service development, new pathways, training, safeguarding enquiries and quality improvement initiatives
  • Contribute to testing and shaping this pilot trusted assessor model and future ways of working
THE SUCCESSFUL CANDIDATEThe successful candidate will have the following skills and competencies:
  • Professional qualification in Nursing
  • Significant post-qualification experience working with older people and adults with long-term conditions in hospital and community settings
  • Experience of working within or alongside hospital discharge teams
  • Strong understanding of adult safeguarding, mental capacity, and relevant legislation
  • Specialist knowledge of the National Framework for NHS Continuing Healthcare and Funded Nursing Care
  • Excellent communication, negotiation and relationship-building skills
  • Ability to work under pressure, manage complex cases and resolve conflict professionally
  • Strong analytical, report writing and data interpretation skills
  • Ability to travel across and outside the borough
  • Willingness to work flexibly, including occasional work outside normal hours
