Unit Description: The Skilled Nursing Facility (SNF) at Champlain Valley Physicians Hospital is located on our 5th floor and accommodates 34 residents. Our facility serves a diverse range of patients, spanning from middle-aged adults to geriatric residents. We maintain a balance between long-term care and subacute care, striving for a ratio of 14 short-stay patients to 20 long-term residents. Unit Description: R6 is a 24 bed Medical Surgical unit. It specializes in Orthopedics, Trauma, and Stroke/Neurology. Included in our 24 beds, we have a 10 bed Joint Care Center, where they have their total joint replacement patients. Because we specialize in caring for shorter stay surgical patients, we tend to have one of the higher discharge/admission rates in our facility. Oncology care ranges from, symptom management, radiation, Chemo (IV and oral), medication side effects and Oncological Emergencies.
