Position Summary
The Advanced Practice Professional (APP) coordinates and applies clinical skills for the comprehensive management of Heart and Vascular Institute (HVI) critical care patients, including biological, social, and psychological assessments. The APP coordinates involvement of medical specialty teams to develop a comprehensive plan, reviews the plan of care with the clinical team, participates in directing care, performs diagnostic/therapeutic procedures, and coordinates discharge planning/follow-up (as applicable). The APP participates in the education of residents and medical/nursing students. Provides healthcare services consistent with state law and licensing requirements; services may include but are not limited to the duties listed below.
Education, Certification, and/or Licensure
Minimum Qualifications For Nurse Practitioner
- Graduate of a CCNE-accredited nursing program with Master's Degree in Nursing.
- National Certification by one of the recognized APRN Certification Centers.
- Licensure or eligibility for licensure as a professional Registered Nurse and Advanced Practice Registered Nurse in West Virginia.
- American Heart Association BLS Certification required with additional Advanced Life Support training at the discretion of the department.
Experience
- Eligible new graduates will be considered.
Preferred Qualifications
- Critical care, cardiac and/or acute care experience preferred.
- ACNPs preferred but will consider FNPs.
Minimum Qualifications For Physician Assistant
- Bachelor's Degree as a Medical/Surgical (as applicable) Physician Assistant required.
- Graduate of an ARC-PA approved Physician Assistant (PA) training program required.
- Certification by the National Commission for Certification of Physician Assistants required.
- Must be licensed or eligible for licensure by the WV Board of Medicine or WV Board of Osteopathic Medicine.
- American Heart Association BLS Certification required with additional Advanced Life Support training at the discretion of the department.
Education, Certification, And/Or Licensure
- Master's Degree as Physician Assistant preferred.
- Critical care, cardiac and/or acute care experience preferred.
Core Duties and Responsibilities
The following describes the general nature of work and is not an all-inclusive list. Other duties may be assigned.
- Manage comprehensive and individualized care for HVI critical care patients and their families during inpatient ICU care and immediate post-transfer phases.
- Complete daily rounds; conduct consults and/or pre-procedure evaluations.
- Perform integrated assessments and with faculty collaboration, order tests or consults as needed.
- Perform and communicate results of diagnostic tests following clinical guidelines and scope of practice.
- Consult and communicate with clinic and hospital staff regarding abnormal findings.
- Perform succinct comprehensive clinical evaluations and document findings in the medical record.
- Facilitate patient discharge or transfer by coordinating services and follow-up tests as applicable.
- Develop a comprehensive, individualized plan of care for HVI critical care patients, including teaching, discharge planning, and referrals.
- Demonstrate expertise in caring for patients across the age continuum and identify unstable or complex critical care situations requiring intervention.
- Recognize emergencies and initiate appropriate emergency care.
- Evaluate critical care patients and coordinate follow-up procedures and tests in ambulatory settings as applicable.
- Contribute to outcome measures collection and clinical decision-making using evidence-based principles.
- Provide education to nursing staff, hospital staff, students, patients, families, and the community; promote learning and adaptation of care processes.
- Update knowledge base and complete clinical competencies; serve as a resource for staff caring for critical care patients.
- Participate in community programs and dissemination of information within area of expertise.
- Document the plan of care, interventions, and evaluations clearly in the patient record following policies and compliance standards.
- Collaborate with leaders and staff to achieve patient outcome-based improvements in care and lead changes in critical care practice to positively impact outcomes.
- Work with medical and nursing staff to solve problems and provide professional support; participate in interdisciplinary approaches to care and education.
- Promote innovation in the expanded role for care delivery in inpatient and ambulatory settings; participate in committees and task forces.
- Document hours and submit billing reports as appropriate; review and interpret critical care data and recommend changes as necessary.
- Participate in clinical trials and research studies with the Principal Investigator when applicable.
- Other duties or projects as assigned.
